1. Information Applicants Must Provide

For a DBS Check, the applicant must provide:

  • Personal details, such as their full name and date of birth

  • A full five-year address history with no gaps

These details are required for all DBS applications.


2. Identity Documents Applicants Must Provide

In addition to personal details, the applicant must provide several documents as proof of identification.

You must use the official DBS document routes and rules.
